SPORTS PHYSICALS

Dr. Gonzales and Dr. Marcri are certified by the Colorado Chiropractic Association to perform Pre-Participation exams for school activities throughout the state. These exams are also applicable for summer camps and other extracurricular activities.

We recommend scheduling exams at least one month before the start of the sport or activity to allow time for any additional testing or specialist referrals if necessary.

VA MUSCULOSKELETAL DISABILITY EXAMS and Community Care Network

We are an approved VA community care chiropractic clinic.  As a VA approved chiropractor in Evergreen, CO we are able to treat veterans with VA healthcare benefits in the mountain community at no cost to the veteran. 

 

The Department of Veterans Affairs (VA) will give a Military Disability Rating for each service-connected condition a service member has.  In order to apply for VA benefits supporting documents need to be collected including service records, medical records and a current exam (DBQ).  The doctor performing the exam needs to be familiar with the process and proper documentation.  The doctors at Evergreen Spine and Sports Medicine (ESSM) are specialists in the examination and evaluation of the musculoskeletal system.  The doctors are also familiar with the required process and evaluation for musculoskeletal disability ratings for the Department of Veterans Affairs. 

 

Here are the steps to scheduling a DBQ exam at ESSM:

 

  1. Gather all service records.

  2. Gather all previous medical records including exams, treatments, imaging (X-rays, MRIs, CTs, Ultrasounds), lab tests, etc.

  3. Call 303-670-8902 or email info@evergreenchiro.com to request intake paperwork for a DBQ exam.  Make sure to indicate how many areas will be evaluated.

  4. Email info@evergreenchiro.com or fax 303-670-3580 with all service records, medical records, and intake paperwork.

  5. The staff at ESSM will call you to schedule your exam.
